How do we act regarding functional diversity?

Evaluation and recognition of functional diversity

  • Anamnesis and interview with the patient and their environment: we collect detailed information about the patient's communication skills and needs, as well as their goals and preferences.

  • Functional assessment of language and communication: we examine the way the patient communicates in their natural environment, taking into account their abilities and areas where they may require support.

  • Identification of strengths and needs: we recognize and value the patient's communicative and functional abilities, as well as the areas where they can benefit from intervention.

Planning of personalized interventions for functional diversity

  • Establishing functional and realistic goals: we define communicative and functional goals that reflect the patient's individual interests and needs, as well as their life context.

  • Design of personalized strategies and adaptations: we develop approaches and techniques that fit the patient's abilities and preferences to facilitate effective communication and participation in daily activities.

  • Promotion of autonomy and independence: we work on communication skills that allow the patient to express themselves and make decisions autonomously, promoting their empowerment.

Multidisciplinary intervention and interprofessional collaboration

  • Teamwork with other health professionals: we collaborate with occupational therapists, physiotherapists, and other specialists to comprehensively address the patient's functional needs.

  • Adaptation of strategies to specific environments: we consider how communicative and functional skills can be applied practically in different contexts, such as at home, school, or work.

Patient progress

  • Monitoring of functional and communicative skills: we perform periodic evaluations to measure progress and adjust the intervention plan as necessary.

  • Incorporation of strategies into daily routine: we provide guidelines and activities that the patient and their environment can practice to reinforce progress in different contexts.

  • Ongoing support and empowerment of the patient and their environment: we offer guidance and resources to the family and the patient to maximize autonomy and quality of life in their daily lives.
